Annual Chronic Illness Reviews
The surgery will be updating the way we contact you regarding your annual chronic illness reviews.
Examples of chronic illnesses include; Diabetes, Chronic obstructive pulmonary disease (COPD), Asthma, Chronic Kidney Disease (CKD), Atrial Fibrillation (AF), Hypertension, Stroke, Transient ischaemic attack (TIA) , Heart Failure, Non- Diabetic Hyperglycaemia and Coronary Heart Disease.
We will now be aligning your annual review date with your month of birth to make it easier for you to remember when this is due.
We are doing this for all patients who require annual monitoring.
If you have multiple chronic conditions, we will be combining your reviews and will invite you for a review for all of these at the same time, whether this be for blood tests or face to face reviews. We hope that this will be beneficial and limit the amount of appointments needed to manage your conditions throughout the year. This will not stop you from being able to contact the surgery if you need advice and help outside of your reviews.
If you require interim monitoring for your chronic illnesses outside of your annual review, for example; an interim blood test, you will be advised and invited accordingly.
We hope this transition runs smoothly but welcome your feedback on this change.
